Pharmacy Quality Scheme 2025/26

The NHS Business Services Authority (NHSBSA) has published the questions which will be included in the 2025/26 Pharmacy Quality Scheme (PQS) declaration so pharmacy owners can view these ahead of making their declaration when the declaration window opens.

The questions are available on the NHSBSA PQS hub page; scroll down to the ‘PQS 2025/26’ section where you can access the questions.

Declaration period

Pharmacy owners must make their PQS declaration between 9am on 2nd February 2026 and 11.59pm on 27th February 2026. You must have evidence to demonstrate meeting the domains that you have claimed for by the end of 31st March 2026.

Patient Safety Incident Reporting

As part of NHS Community Pharmacy Contractual Framework and guidance from PREVIOUS Pharmacy Quality Scheme (PQS), patient safety reports should be analysed and discussed with the pharmacy team on a monthly basis.

  • Monthly Analysis: It is highly recommended that pharmacies use a monthly template to review patient safety incidents, which helps in identifying trends and learning.
  • Team Discussion: These monthly reports should be shared with the pharmacy team to facilitate learning and encourage improvements in practice.
  • Annual Requirement: While monthly reviews are required, an Annual Patient Safety Report must also be produced to consolidate the year's findings, which is used for evidence during inspections.
  • Immediate Action: Note that while the formal report is reviewed monthly, individual near misses and errors should be reported and discussed immediately as part of standard operating procedures (SOPs)

More information can be found on the CPE website.

The World Pharmacy Council's Annual Sector Analysis Document

The World Pharmacy Council has published its annual sector analysis document, which pulls together data from across the member countries to allow comparison across countries and learning from each other.

The public version of the report is now available on the WPC website and this may be of interest to you.
