Preparing to provide the Discharge Medicines Service

To support contractors preparing to provide the Discharge Medicines Service (DMS), which will become an Essential service for contractors on 15th February 2021, PSNC has published some additional resources on the DMS page of our website.

These additional resources include:

• A DMS Contractor checklist to assist contractors to work through the actions needed to get ready to provide the service;
• A Briefing sheet: Introduction to the DMS for general practice and PCN clinical pharmacy teams, to provide contractors with a short introduction to the service that they can use to engage their local GPs and clinical pharmacists; and
• A DMS briefing for pharmacy teams to help contractors brief their pharmacy team members on what the DMS service entails.
