Livewell Plymouth – Introducing Self Referrals to Podiatry
Livewell is pleased to announce that from today, Wednesday 27 September, potential new patients, or their carers and relatives, can now refer themselves to Podiatry services if they are registered with a GP in the Plymouth, South Hams and West Devon areas.
To fully inform you of this process you will find attached to this email an Information Sheet for Health Professionals which details how self-referrals can be made as well as the other support available to yourselves and the public via our self-referral website.

A patient information leaflet which explains the process for potential patients, and a poster you may wish to display in public facing areas.
